  • What documents do I need for an account transfer or rollover?

    You will need to provide the following,

    1. Recent statement for the account being transferred
    2. Two pieces of ID
    3. Copy of the form that requires the medallion signature guarantee (complete the form but do not sign it)
  • What documents do I need for a death related transfer?

    Documents needed for a death related transfer include the following,

    1. Death certificate
    2. Proof that you’re the beneficiary, trustee, executor or co-owner of the account
    3. Two pieces of ID
    4. Copy of a recent statement or other documentation showing the $ value of the transfer
    5. Copy of the form that requires the medallion signature guarantee (complete the form but do not sign it)

  • What documents do I need for a Treasury Direct?

    -Copy of the statement or statements that show the value of the bonds you will be redeeming (Only if transaction involves a redemption of treasury securities)


    -Two pieces of ID (drivers license or passport + 1 other)


    -Copy of the treasury form that requires the medallion stamp


    What you need to do:

    1. Provide us with the documentation detailed above. You can either email to: documents@verifymysignature.com, or you can securely upload the documents using the following link: (link: https://www3.mydocsonline.com/cuploadcustom.aspx?id=vmsmedallion)
    2. Visit: https://www.verifymysignature.com/shop)and select the $149 option to purchase your medallion signature guarantee stamp. Complete the purchase
    3. You will receive an email from us with various available time slots to complete the required Zoom video conference
    4. Once you agree to a time slot, you will be sent a Zoom video conference link to join the conference in advance of the scheduled time. The conference normally takes approximately 5 minutes
  • What documents do I need if I have inherited stocks, bonds, cash, or other assets?

    You will need the following

    1. Most recent account statement (if applicable)
    2. Testamentary letter or other document confirming Executor or Trustee status
    3. Drivers license of Executor or Trustee
    4. 1 additional piece of ID from Executor or Trustee
    5. Share Certificate or Ownership Statement
    6. Stock Power (this document requires medallion stamp)
